AtlantiCare’s Terri Schieder, RN, MS, MBA, and

Gavi Degapudi, M.D. discuss care transitions at

Premier Population Health Management Collaborative

 

AtlantiCare’s Terri Schieder, RN, MS, MBA, vice president, Population Health and Business Development; and Gavi Degapudi, M.D., medical director, Care Transitions and Hospice, shared best practices at the Premier Population Health Management Collaborative annual meeting today.

During a general session at the Sheraton Pentagon City Hotel in Arlington, Virginia, Schieder and Degapudi discussed the importance of care transitions and post-acute care networks. Among the tips they shared were:

  • Have a multi-disciplinary team approach to ensuring safe, effective transitions across all healthcare settings
  • Identify patients who are at high risk for complications and provide appropriate care
  • Have good, clear, timely communication among all healthcare providers
  • Address end-of-life issues/planning proactively, compassionately, and inclusively
  • Use technology and other tools to track, monitor and engage patients
  • Evaluate, and reevaluate trends and clinical outcomes regularly

Gavi Degapudi, M.D., medical director, Care Transitions and Hospice, AtlantiCare, discussed how AtlantiCare works with providers, patients and staff to encourage end-of-life care discussions and planning. She was among presenters at the Premier Population Health Management Collaborative annual meeting in Arlington, Virginia.

 

Premier Population Health Management Collaborative

Premier’s Population Health Management Collaborative helps providers meet the challenge of improving the health status and care experience for their patient populations. Hospitals and health systems participating in the Population Health Management Collaborative work to connect different elements affecting care delivery. Medicare ACOs participating in the collaborative have outperformed other Medicare ACOs nationally for the past three years. We have years of experience and have developed proven methodologies around what works and how to stage an ACO.
