AtlantiCare LIFE Connection

Program of All-inclusive Care for the Elderly

opens in Atlantic City

 

AtlantiCare, a member of Geisinger, has opened a Program of All-Inclusive Care for the Elderly (PACE®). PACE integrates preventive, primary, acute, rehabilitation and long-term care services with the goal of providing care timely and in the most appropriate setting. The program is designed to offer all-inclusive care, 24 hours a day, 365 days a year. There are no benefit limitations, co-pays, or deductibles for those who are dually eligible through Medicare and Medicaid who use PACE services. As a PACE organization, AtlantiCare serves as provider and insurer for participants.

“We are pleased to offer this new, innovative service that will make a difference and change people’s lives,” said Lori Herndon, president and CEO, AtlantiCare at staff reception marking the opening of the program. “Our team will wrap its arms around all participants to help them have the most productive, satisfying experiences in the twilight of their lives.”

AtlantiCare LIFE Connection (Living Independently for the Elderly) is based in the William L. Gormley AtlantiCare HealthPlex in Atlantic City. The 7,000 square foot facility includes an activities area, serving kitchen, dining room, quiet lounge, primary care clinic with three exam rooms, personal care area, and outdoor courtyard. Through AtlantiCare’s Healing Arts Program, the site features than 30 pieces of New Jersey artists’ work depicting landmarks in Atlantic City and surrounding areas.

The center is open Monday through Friday from 8 a.m. to 4:30 p.m. There is 24/7 access to healthcare services and support. It is designed for those individuals who have long-term care needs and who wish to remain in their communities.

AtlantiCare LIFE Connection is available to those individuals living in Atlantic or Cape May counties and who are at least 55-years of age. Applicants must also be deemed eligible for nursing facility level of care, and be able to live safely in the community with PACE services.

“We refer to those who use this service as participants rather than patients, because we partner with them in their home setting and through our Day Center.” said Richard Stefanacci, D.O., medical director of AtlantiCare’s Post-Acute Services and LIFE Connection.

The AtlantiCare LIFE Connection team includes physicians, nurses, social workers, dietitians, physical and occupational therapists, drivers, personal care attendants, and other providers and staff.  “Core team members will meet weekday mornings at the Day Center to discuss the care needs of all participants,” said Stefanacci.

“Our team works with each participant and his or her family and caregivers to create a personalized care plan, said Alex Casiano, executive director for LIFE Connection. “We want these individuals to truly participate in their health and wellness. We coordinate and provide all health care and support services, including medication and/or other therapy for a chronic or episodic condition. We also assist with activities of daily living so the participant can continue to live safely at home.” Casiano added all services must be pre-approved by the LIFE Connection interdisciplinary team.

Giovanni Avelluto, D.O., AtlantiCare Physician Group, will be on site at AtlantiCare’s LIFE Connection Day Center at the William L. Gormley AtlantiCare HealthPlex, Atlantic City.

AtlantiCare LIFE Connection is the sixth PACE program to open in New Jersey and the only one to serve Atlantic and Cape May counties. It is a member of the National PACE Association.
