AtlantiCare to participate in week-long observance of
National Healthcare Decisions Day

 

Marking National Healthcare Decisions Day (NHDD) – April 16, AtlantiCare, a member of Geisinger, is promoting the importance of advance care planning with a week-long educational campaign from Monday, April 16 through Friday, April 22. The theme for 2018 NHDD is “It always seems too early, until it’s too late.” AtlantiCare’s Advance Care Planning Team will provide patients, members of the community, and staff with information about and assistance with advance care planning at the following locations:

Certified advance care planning facilitators will cover the importance of planning for end-of-life care; how values and beliefs guide decisions about care; what a healthcare agent is and does; what advance directives and other advance care planning documents are, and how to complete and use them; and more.

“Advance care planning allows individuals to clearly outline their personal choices about treatment options that take into account their personal values, beliefs, and preferences,” said Christeen Cornell, MSN, RN, CCM, director, AtlantiCare Advance Illness Management. “A common misbelief about advance care planning is that only older or very sick people need written healthcare directives. However, life can change in a moment for anyone at any age. Traumatic accidents and sudden serious illnesses can happen, even to young people and those in the prime of life. All adults, 18 and older, should have an advance directive that they share with their loved ones and their healthcare providers”

Through AtlantiCare’s Advance Care Planning service – My Health, My Wishes, My Choice, AtlantiCare offers patients, their families, members of the community, as well as providers and , advance care planning information and assistance. The Advance Care Planning Team shares information with groups, organizations, and businesses through its Conversations Matter: Advance Care Planning community lecture by request. AtlantiCare also provides information, including videos and links to certify with patients and their supporters to discuss advance care planning.
