AtlantiCare WOW ME 2000mg tool for heart failure helping patients manage symptoms, stay out of hospital
Now there’s an app for that
AtlantiCare has created a free app to assist people in managing heart disease. The WOW ME 2000mg app, available on the App Store (https://itunes.apple.com/us/app/wow-me-2000mg/id576776422?ls=1&mt=8), is based on a paper-tool ARMC’s Heart Failure Resource Center Team created and has used since 2010 that has resulted in reduced number of hospital admissions and readmissions for heart failure.
A team led by Shannon Patel, RN, AtlantiCare Heart Failure Program manager, developed the tool. “Our goal was to help patients who are in any stage of heart failure, their caregivers and family members, identify and manage symptoms and avoid preventable hospitalizations,” said Patel.
The WOW ME 2000 acronym reminds users to daily:
· Weigh themselves
· Measure their Output of fluids
· Walk/be active
· Take their Medications appropriately
· Evaluate signs and symptoms of
· Limit salt to 2,000 mg or less, with 1,500 mg being optimal
The Heart Failure Resource Center and AtlantiCare Information Technology teams collaborated on research and development of the virtual version of the WOW ME 2000 tool in 2011 and 2012; trialed it with physicians and other clinical professionals in 2012 and sent it to Apple in 2012. The app was officially available for download as of January 14, 2013.
The app prompts users with reminders and allows them to enter information about how they are managing symptoms. It also links them with the AtlantiCare’s Heart Failure Resource team and other providers
“In the Pennsylvania New Jersey region, one out of five heart failure patients is readmitted to the hospital within 30 days of discharge,” says Howard Levite, MD, medical director of the Heart Institute at AtlantiCare Regional Medical Center and of ARMC’s Cardiac Catheterization laboratories at ARMC’s Cardiac Catheterization and Rhythm Center and president of the Board of the American Heart Association Central/Southern New Jersey Region. “A major reason is that patients don’t have the support they need after they leave the hospital or the education to manage their heart disease.”
Early results of AtlantiCare’s WOW ME 2000mg tool, attained prior to the virtual version, include:
· Decrease in total volume of heart failure-related admissions to the hospital
· Decrease the readmission of our heart failure readmissions within 30 days
· Decrease in readmission of all-cause admissions within 30 days
“We have achieved the goal of this self-management tool by reducing preventable readmissions for heart failure and to improving the quality of life for our patients,” said cardiologist Viren Vankawala, MD, AtlantiCare Physician Group Cardiology. “The extension of this tool, AtlantiCare’s WOW ME 2000 app, is an additional convenience for our patients. It is also a tool that others managing heart disease, regardless of where they live or are being treated, can use with their care providers and families.”

AtlantiCare opened its Heart Failure Resource Center, based at the Heart Institute at ARMC Mainland Campus, and with additional locations in the community in Pomona in 2010. It earned the Joint Commission’s Heart Failure Certification in 2010 and its Advanced Heart Failure Certification in 2012.
