Advance Directive for Health Care (Living Will) English Version
An advance directive is written legal document that tells your doctors how you want to be treated, in the event you become incapacitated. It also designates a healthcare proxy (spokesperson), someone who will make sure your wishes are followed if you can’t express them yourself.

Five Wishes is considered a legally valid living will, written in everyday language that makes it easy to understand and complete. It covers your personal, spiritual, medical and legal wishes in a single document.
