Getting Started:  There are two ways to join!!

Option 1 
During your visit, ask the front desk for help setting up an account. 
You will need to use your computer, smartphone or mobile device to access the Internet, and will follow the steps on the website the front desk directs you to.

You will create a password and username other than your E-mail address before using HealtheLife for the first time.

Option 2 * 
Self enroll by typing the following link into your web browser and following the steps listed there.

To maintain the privacy and security of your personal health information, do not share this password with anyone else. 
*Must have previously provided a valid email to your provider

With the patient portal, you can…

• Check your personal records anytime, anywhere using a computer, smartphone, or tablet!
• Receive email notifications when your provider responds to your message or reaches out to you directly
• Request prescription renewals
