The program is overseen by board-certified rheumatologists who specialize in caring for joints, muscles and bones. They have decades of cumulative experience and, using state-of-the-art diagnostic and treatment technologies, will work together with specialists from other fields to help look after and restore your bone health.

INTERDISCIPLINARY
Osteoporosis is a disease that requires a skilled management team to monitor, plan and coordinate how to keep your bones at their best for strength and mobility. You may first be referred for screening by your primary care provider, OB/GYN or rheumatologist. These providers will then stay involved with your care — in collaboration with a full interdisciplinary team — to build a prevention and treatment plan, and walk with you every step of the way through your osteoporosis care.

- RN post-fracture care liaison, program coordinator

What did we do to earn it?
In short, our continuum of care. We’re completely dedicated to the bone health of our community and go to great lengths to make sure that potential osteoporosis cases are detected as early as possible, so no matter what type of care our patients receive, their bone health is always prioritized.
The Fracture Liaison
Our Fracture Liaison is integral to our early intervention process. This is a special clinician who identifies fracture patients whose injuries may have resulted from weakened bones. They are immediately referred to a rheumatologist and entered into the osteoporosis care pipeline, so that we can begin working to restore their bone health and prevent further fractures.
Who should be tested for osteoporosis?
Too often, osteoporosis goes undetected and untreated until after a fracture occurs. But a bone-density screening can detect it early, allowing us to begin treatment and prevent a fracture or other complications.
According to the National Osteoporosis Foundation, the following individuals should be screened:
- Women age 65 or older
- Men age 70 or older
- Broken bones after age 50
- Women of menopausal age with risk factors
- Postmenopausal women under age 65 with risk factors
- Men age 50 to 69 with risk factors
Talk with your provider to see if they’d recommend a bone-density screening.
