Schedule an AppointmentAtlantiCare Regional Medical Center’s Outpatient Anticoagulation Clinic is operated by registered and specially trained Medication Therapy Disease Management (MTDM) pharmacists who will work with your primary care provider and healthcare specialists to help manage the chronic disease medication regimens for high-risk chronic diseases or anticoagulation therapy. Our goal is to provide safe, effective, and convenient management of your injectable and/or oral anticoagulant medications such as warfarin (Coumadin®).
Anticoagulants, commonly known as “blood thinners”, are prescribed to prevent and treat a variety of blood clot conditions. Proper management of these medications is important in order to maximize efficacy and minimize risks of adverse events such as bleeding.
AtlantiCare MTDM pharmacists will work with you and your healthcare providers to come up with a customized medication management plan that fits your daily routine and lifestyle. For many, the support of the MTDM pharmacist is life-changing.

- Face-to-face patient education
- Point-of-care (finger-stick) testing
- Close supervision of treatment for effectiveness and monitoring for side effects
- Adjusting therapy around surgery or other procedures

- List of all prescriptions, over-the-counter medications, and any herbal or complementary medications
